Behavior Framework
  • High School/Middle School Referral - This is the referral form for the Wellpinit High School and Middle School. This form is meant to track concerning behaviors and guide administration in taking appropriate steps to correct the behavior and reteach the PRIDE expectations.
  • Minor Incident Report - This form is used at the Elementary School to document student behavior that while minor, begins to show a trend. Once a student has three reoccurring behaviors documented, this form is turned into the office and documented using the referral form. This means the behavior is now become office managed.
  • Minor and Major Behavior Explanations - This form is a guide to parents, teachers, staff, and students to which behaviors are considered minor behaviors, those able to be discussed and handled within the classroom, and those considered major behaviors which require office intervention.
  • Elementary Referral - This is the referral used in the elementary school to document concerning student behavior. Refrrals are not punishment but rather a means to track trends in student behavior. The referrals are written by teachers and staff and reviewed by the principal. After which, the data is inputted into the online system and reviewed by the PBIS site team. This data is used to help inform decisions around policy and procedures that affect students during the school day.
